Minute-by-minute: The Deepwater Horizon's final hours

For every security, safety and risk manager who reads SecurityInfoWatch.com, I want to recommend you read an article. It's not on SIW, but it is quite possibly the best article about safety and risk management I have read in years.

On Dec. 26, 2010, the New York Times published the article "Deepwater Horizon's Final Hours." It is a blow-by-blow account of the safety failures, the warning system failures, and the failures to follow safety/risk procedures that led to 11 dead employees and many more injured. If you think your security of safety manual is the "Bible" of how you will operate in a crisis, think again...

I can't recommend this article enough for security, safety and risk managers. It will make you rethink training, rethink chain of command, rethink decision-making...
